Caffeine and Performance: Dose, Timing, Tolerance

Caffeine is the most widely used performance-enhancing substance in sports. The International Society of Sports Nutrition's 2021 position stand — authored by Dr. Nanci Guest at the University of Toronto and 14 co-investigators — quantified the effects across 21 meta-analyses: endurance improves by 2-4%, strength by 2-7%, and power output by 3-8%. The International Olympic Committee removed it from the banned substances list in 2004.
The optimal dose is 3-6 mg per kilogram of body weight, taken 30-60 minutes before exercise. Dr. Guest's research on the CYP1A2 gene shows that roughly 50% of the population are fast metabolizers who respond best at the lower end (3 mg/kg), while slow metabolizers may need closer to 6 mg/kg. Below 3 mg/kg, effects are inconsistent. Above 6 mg/kg, side effects — anxiety, GI distress, tachycardia — increase without proportional gains. More is not better.
Timing matters because caffeine reaches peak blood concentration 45-60 minutes after oral ingestion. Taking it too early means levels are declining during your session. Taking it too late means you start training before the full effect kicks in. Set a 45-minute timer.
Tolerance reduces but does not eliminate the performance benefit. Habitual coffee drinkers still see a 1-3% performance improvement from pre-exercise caffeine, compared to 3-5% in naive users. However, a 7-day caffeine washout restores full sensitivity, which is why some athletes cycle off before competitions.
The form does not matter much. Coffee, caffeine pills, pre-workout powders, and caffeinated gum all deliver the same molecule. Coffee has the advantage of additional polyphenols and chlorogenic acids with anti-inflammatory properties. Pills have the advantage of precise dosing.
One underappreciated effect: caffeine reduces perceived exertion by 5-6% without changing actual physiological strain. You are not working harder — you just think the work is easier. This psychological effect is real and measurable, and it explains why caffeine improves performance across such a wide range of activities.
